SERVICE SPECIALIST – PERRYSBURG, OH
Full-time, Monday – Friday, typically 7:30 AM – 3:30 PM
We are seeking a Service Specialist to join our team. This role involves working directly with individuals, guardians, families, and County Boards to coordinate and attend ISP meetings, ensuring compliance with RVI services. The ideal candidate will be responsible for gathering and managing data for outcome tracking, verifying ISPs for accuracy, and communicating key information across teams.
Key responsibilities include documentation management, professional training, skill development planning, and ensuring compliance with regulations (DODD, CMS, OOD). The role also involves assisting staff with documentation, responding to referrals, and overseeing safety and compliance procedures. Strong communication skills, attention to detail, and a commitment to trauma-responsive care practices are essential.
Requirements:
• High school diploma or equivalent.
• Habilitation programming for individuals with DD.
• Ability to complete mandatory DD certification classes.
• Completion of mandatory Blood Bourne Pathogens, CPR, and First Aid training.
• Deal with problems involving several variables in familiar context.
• Maintain accurate records.
• Willingness to work with and cooperate with coworkers.
• Ability to perform personal service duties, including but not limited to, feeding and bath rooming.
• Valid driver’s license.
• Excellent written and verbal communication skills.
• Excellent organizational skills and proficiency in professional office procedures.
• Prefer 2 years’ experience in DD field, equivalent education, or combination of both.
